Milan are missing several key players and Santiago Gimenez joins Joao Felix upfront, while Roma must look to Eldor Shomurodov and Matias Soulé.
It kicks off at the Stadio Olimpico in Rome at 19.45 UK time (18.45 GMT, 20.45 CET).
There is still so much at stake here, as Milan must shake off the disappointment of Wednesday’s 1-0 Coppa Italia Final defeat to Bologna, which was also their best hope of qualifying for Europe.
Roma dropped to sixth place with their loss against Atalanta, so are two points behind fourth, but also only three clear of Milan in eighth.

Both teams are struggling badly with absentees for this fixture.
Artem Dovbyk is ruled out by muscular problems, while the season is already over for Paulo Dybala and Lorenzo Pellegrini.
Shomurodov is therefore the only available centre-forward with Soulé pushed into a more advanced role.
Alexis Saelemaekers faces his parent club and Leandro Paredes steps into the midfield.

Milan have made the trip without Theo Hernandez, Kyle Walker, Samuel Chukwueze and Warren Bondo, but Riccardo Sottil is back in the squad.
Alex Jimenez therefore shifts over to the left flank, bringing in Yunus Musah on the right and Ruben Loftus-Cheek in midfield.
Gimenez is preferred to Luka Jovic and Tammy Abraham, flanked by Christian Pulisic and Joao Felix, as Rafael Leao and Youssouf Fofana drop to the bench.
