Liverpool suffered just their second defeat under Arne Slot as Lucas Bergvall scored the only goal in the Carabao Cup semi-final first leg in north London.
The first half was dominated by stoppages, with Rodrigo Bentancur stretchered off with a worrying injury before Jarrell Quansah was also forced off with a problem.
As a result, neither side were able to grab the upper hand heading into the break, with Antonín Kinský looking assured on his Spurs debut.
Tottenham came close to opening the scoring just before the hour mark when Lucas Bergvall forced Alisson into a mistake, only for Pedro Porro to fire wide with the goal at his mercy.
With 20 minutes left to play, it was Tottenham's turn to survive a scare as Radu Dragusin cleared an effort from Trent Alexander-Arnold off the line.
Spurs thought that they had snatched a lead late on when Dominic Solanke ran past the Liverpool defence to slot past Alisson, only for VAR to rule it out for offside, a decision that was broadcast by the referee to the 60,000 Spurs fans in a historical moment.
With five minutes left to play, Lucas Bergvall escaped a potential second yellow card for a late challenge on Kostas Tsimikas, which came back to bite the Reds less than a minute later as the Swedish midfielder swept home from 12 yards out after excellent hold up play from Dominic Solanke.
Liverpool pushed for an equaliser and Kinský was forced into a fine save to deny Darwin Núñez.
But Spurs held firm for a famous win, taking a 1-0 lead to Anfield for the second leg in a month's time.
